Voucher – Terms and Conditions

• Nonredeemable for cash or transferable
• NO Treatment included
• 1 voucher per person
• Payment made at time of booking
• The “Back Pain Assessment” includes any chiropractic, Neurological, and Orthopedic tests requires on the day.
• If specific spinal x-rays are required they will be provided onsite on the day, only charged to health fund if applicable.
• If any other test are required, i.e MRI, CT scans these will be arranged off site.
• A Full Report of Finding is included if required
• Chiropractor has right to refuse services
• If appointment is booked and missed without 24 hours notice Voucher is void
